Union Square is pleased to announce that the Seattle Visiting Nurse Association (SVNA) will be providing a Flu Immunization Clinic at Union Square. The SVNA is a community based, not for profit, Washington State Corporation.
2019 flu immunization clinics are scheduled for:

The clinic is open to all occupants of Union Square. Participants will need to complete the waiver of liability and notice of privacy practices, which are attached. The flu shots are administered by registered nurses. SVNA is able to bill most primary health plans – Regence, Group Health, Premera, United HealthCare, First Choice and Aetna. They do not accept Cigna or straight Provider One unless the plan is tied to an Apple Health Plan. You may also pay by cash or check; payment is $40.00 for standard Quadrivalent, due at the time the shot is given. The nurses also carry a small amount of Thimerosal-Free vaccine for pregnant women as required by Washington State regulations.
For those employers who wish to pay for their employees’ flu shots, SVNA can invoice upon completion of the clinic. Please provide SVNA with billing information (to include the contact name at your company and a list of employees receiving the shot) on your company letterhead on the attached roster billing sheet. They will bill at the completion of the clinic, but the billing information and a list of employees must be provided prior to the clinics. Email to Tenant Services at email@example.com.
